I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS EVENT, and WAIVE, RELEASE AND DISCHARGE Events by M, Ltd., and their directors, officers, employees, volunteers, representatives, and agents, hereby referred to as “EbM”, from any and all liability, including but not limited to, liability arising from the negligence or fault, death, disability, personal injury, property damage, property theft, or actions of any kind which involve Event Participants, including travel to and from the Event.

I acknowledge that Events by M is NOT responsible for errors, omissions, acts, or failures of any party or entity providing a specific event, activity, service, meal, transportation, or lodging which may result in, but not limited, to illness, injury or death.

Commvault, Arrow, Vast, Everpure & Events by M assume no responsibility and are not liable for the following, but not limited to:

Any occurrence associated with consumption of food & beverage, not limited to, but including food poisoning, illness, injury and/or death;
Any incident while under the influence of alcohol including but not limited to unlawful behavior, accident, illness and/or death.
I represent that I am at least 21 years of age and am prepared to present a government-issued identification card, evidencing my age, consistent with applicable legal requirements.
Participation and/or acceptance of merchandise is not mandatory and is of my own free will.
I agree to the use of my contact information including but not limited to email, phone number and address solely for the purposes of coordinating shipment(s) related to this event.
I consent for Events by M, Ltd. and their contracted associates to ship any necessary items associated with this event to the shipping address I provide in registration.

Release of Liability – Physical Activity

I am physically fit to participate in this in-person event.
I understand and acknowledge the dangers and risks inherent in activities related to physical exercise.
I am voluntarily participating in the physical activity related to this in-person event.
I understand that there are risks associated with my participation in this activity, such as physical and/or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability or death. These injuries or outcomes may arise from my own or other’s actions, inactions, or negligence, or the condition of the activity location(s) or facility(ies). Nonetheless, I assume all risks of my participation in this activity.

Commvault, Arrow, Vast, Everpure & Events by M are not responsible for change fees, no shows, schedule changes, cessation of business, or weather cancellation/delays.

I understand that I must comply with the venue's Admittance Guidelines and that non-compliance with said terms, restrictions and guidelines may result in removal from the EVENT. I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the Entities from any and all liabilities or claims made as a result of participation in this EVENT whether caused by the negligence of release or otherwise.

This accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

This shall be effective through December 31, 2099. If any part of this agreement is not enforceable, the remainder of this agreement shall remain enforceable.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT IN WHICH I AM GIVING UP LEGAL RIGHTS AND SIGN IT OF MY OWN FREE WILL.
